Listed below are many forms needed for middle school registration.  The Curriculum and Planning Guide gives detailed descriptions of courses as well as other enrollment information.  The Elective forms are specific to each year.  If your child is in 6th grade in the Spring, they will need a 7th grade elective form for the Fall.
